With a 5-generation difference and the distinction between a flagship and an entry-level model, the HERO12 Black and HERO (2018) have significant differences.
In terms of shooting performance, the HERO12 Black supports resolutions up to 5.3K and offers high frame rates exceeding 60fps at any resolution. In contrast, the HERO (2018) only supports Full HD/60FPS. This results in a substantial difference in video quality.
Regarding shooting features, the HERO12 Black is equipped with high-performance stabilization called "HyperSmooth 6.0," which includes features like horizon leveling. On the other hand, the HERO (2018) only comes with standard stabilization.
The HERO12 Black is a model with enhanced professional features, including a Log shooting mode that supports 10-bit color. These features are not available on the HERO (2018).

The HERO12 Black, with its enhanced professional capabilities, supports wireless voice control via Bluetooth audio and "wireless synchronization of multiple HERO12s using timecodes." The former allows hands-free operation during shooting, while the latter supports efficient post-production workflows when using multiple GoPro cameras.
While these features may not be particularly significant for general users, it's worth noting that the HERO (2018) does not include them.
However, both models do support basic voice control functionality.

Another notable difference is that the HERO12 Black features a built-in folding mount. This allows the camera to be directly mounted to accessories without requiring a case, whereas the HERO (2018) needs a case for such operations.
Additionally, the HERO (2018) does not support loop recording or live streaming, which are available on the HERO12 Black. The supported microSD card size also differs significantly, with the HERO12 Black accommodating up to 1TB, while the HERO (2018) is limited to 128GB.
